Welcome back to another school year! We are
excited for this year and we all hope that each of you
is also.
Forms and Documents
To help ensure a cooperative and successful school
year, we've provided a few key forms and documents on
this page. Please read through them (and return
the forms to the school office by the date indicated).
The following medical forms are due in the school
office no later than the first day of school.
Students without current health and immunization records
on file in the office will not be allowed to attend
school.
The following forms must be signed and returned to
the school office by September 13th. Students may
lose recess if the signed forms are not returned.
Additional Forms
Home and School Association
The purpose of the Home and School Association (HSA)
is to bring into closer relation the parents / guardians
and faculty so that they both may better communicate,
cooperate in, and enhance the educational experience of
the children.
The objectives of the HSA are to:

| donate their time and effort
in the support of fundraising events and school
activities |
Parent(s) / guardian(s) of a child enrolled at
Annunciation Catholic School are automatically placed on
the HSA membership roster and
assessed annual
dues. In addition, each family is required to
provide at least 20 hours of service to designated
activities.
The following membership invoice (with payment)
and volunteer sign-up form must be returned to the
school office by October 15th.
Room Parents
The role of Annunciation Room Parents is to work
closely with the Home and School Association Board and
teachers assisting in various school projects, events,
and fundraisers. Their responsibilities include, but are
not limited to, promoting HSA fundraisers and events;
communicating with parents/guardians; and planning
parties or classroom activities.
