Recommendation Forms
For Incoming Students in
Pre-Kindergarten through Grade 1
For incoming students in Pre-Kindergarten through
Grade 1, one confidential recommendation form should be
submitted to the principal/director at the applicant's
current school. This form is to be completed by the
applicant's current teacher. The applicant's current
teacher is asked to evaluate the applicant’s social,
physical and pre-academic skill development. The
recommendation form provides one way of getting to know
the child and is reviewed with the full awareness that
young children are constantly changing and developing.
The school should mail or fax the completed form
directly to Annunciation Catholic School as soon as
possible, but no later than February 1.
You will be provided with the necessary
recommendation forms by the school office after you have
submitted a complete
application form.
For Incoming Students in Grades 2 through 7
For incoming students in Grades 2 through 7, two
confidential recommendation forms should be submitted to
the principal/director at the applicant's current
school. The applicant's current Math and English
teachers should each complete a recommendation form. The
applicant's current Math and English teachers are asked
to evaluate the applicant’s personal and academic
characteristics. The school should mail the complete form
directly to Annunciation Catholic School as soon as
possible, but no later than February 1.
You will be provided with the necessary
recommendation forms by the school office after you have
submitted a complete
application form.
For All Incoming Students
For all incoming students, Pre-Kindergarten through
Grade 7, a letter of recommendation from the applicant's
priest or minister is strongly encouraged. The priest or
minister should mail the letter directly to Annunciation
Catholic School as soon as possible, but no later than
February 1.
These letters become part of the student’s
application and are reviewed by the Admissions Committee
including the Homeroom teacher prior to the applicant’s
shadow visit. These confidential letters provide
invaluable information about the student on an academic
and personal level. |
